Intra-operatively

The large hernia sac as seen sitting on the base of the right hemithorax at thoracotomy.

The hernia sac wall is made of two layers: parietal pleura and peritoneum.

Omentum is the abdominal content that was herniated into the thoracic cavity,  as we found after opening up the wall of the hernia sac.

One can compare the hernia's size with:

the hand of the operator's assistant (left-below in the photograph)

bullet

the ribs' retractor or "ribs' spreader" (centre-below in the photograph)
